The Practice Of Dr. Rochelle Slamovich Has Merged with Looksie Optometry

I’m not renewing my lease at 450 Sutter. Instead of just shutting down, I’m merging with Dr. Amy Tran’s practice (Looksie Optometry at 1314 Polk Street, between Bush and Pine).

Running an optometry practice, like many small businesses, requires wearing many ‘hats’ besides eyecare, and after almost forty years, though I’ve enjoyed the challenges and rewards of that multitasking, I opted to simplify my work life.  With this  exciting opportunity, I have the luxury of concentrating on patient care and leave the business aspects to the staff at Looksie.

Dr. Tran (as you can see from her reviews) is a well regarded practitioner by patients and the medical eye community. You will be in good hands with Dr. Tran and her friendly team. In addition, Looksie Optometry carries an amazing selection of eyewear from independent designers handcrafted in Japan and Italy. You can check out their selection through their online store.

When am I starting at Looksie Optometry? My projected start date is July 20th.  I’ll be working Tuesdays and Wednesdays. For those of you who want to come other days or on Saturdays, you will be in excellent hands with Dr. Amy Tran.

What happens to my records? Records will be stored off site in a medical records facility. Before you come in for a scheduled eye exam at Looksie Optometry, they will be pulled and delivered there (let the staff know that you are a previous patient from my practice so they can do that.) If you want to have the physical records yourself, there will be a small fee to cover the costs of pulling and mailing them to you. 

How do I book an appointment at Looksie? You can book an appointment online by going to their website or calling their office directly at 415-593-5348. Please mention that you were a patient of Dr. Slamovich so they can prepare your records for you.

Is there parking there? Yes, there’s street parking available on Bush Street and a parking garage on Bush Street (Bush Street & Polk Street).  

Do they take my insurance?  Yes, they take VSP, Eyemed and Spectera. In addition, they take most medical insurance plans. You can go here for more details.

How do I order contacts and glasses after June 25th? You can email Looksie, admin@looksieoptometry, to order contact lenses or eyeglasses and indicate that you’re a patient from Dr. Slamovich’s practice so they can pull up your prescription records.
